Appointments for Enrollment Services – Effective December 12, 2011

To make an enrollment appointment at any of our clinic sites:
Please call 303-602-2310.  Appointments are booked for the next business day.  This appointment line opens at 2 pm daily and remains open until all next day appointments are filled. 

To make an enrollment appointment at Pavilion M, 723 Delaware Street:
Please call 303-602-2300.  This appointment line is open daily from 8:00 am to 5 pm.  Appointments are booked for up to 5 days from when you call. 


 
Enrollment Services has Moved

The Enrollment Services offices previously at 990 Bannock and 645 Delaware are now located on the 3rd floor of Pavilion M at 723 Delaware Street.

 
New requirements to prove residency for CICP and DFAP applicants

If you are applying for CICP or DFAP at Denver Health, there are new requirements to prove that you live in Denver County. 

There are also new requirements to qualify for the CICP or DFAP Z rates. 
 
One Important Change
If you do not own or lease a residence of your own, an affidavit must be completed by the person that you are either living with or who is providing you with a place to receive mail, shower and/or stay occasionally. All affidavits are available from an Enrollment Specialist or on the web at denver health.org under the Enrollment site. 
 
This affidavit must be notarized to be used as proof of Denver residency or homelessness. In addition, documentation that the person who signs the affidavit lives in Denver County must be provided.

Enrollment Services

Denver Health’s Enrollment Specialists can help you sign up for a medical assistance or discount program for which you and/or your family members qualify.

The enrollment specialists will screen you for Medicaid, Child Health Plan Plus (CHP+), the Colorado Indigent Care Program (CICP), the Denver Health Financial Assistance Program (DFAP).


To be eligible for a discount, you must be screened by an Enrollment Specialist. This screening will find out if you qualify for any of the above programs so you can receive a discount on many of the hospital, clinic and pharmacy services at Denver Health.

In order to complete the screening process for all of these programs, you must bring documentation of your current income and expenses, family members, housing status and several other items (see Required Documents).
 
As stated on the application that you will sign, falsifying information on family size, income, housing status (homelessness) or home location is considered fraud and must be reported to the state of Colorado.
 
If you live outside of Denver County, you can still get care at Denver Health if you qualify for Medicaid or CHP+. You MUST live in Denver County to receive non-urgent care at Denver Health under CICP and DFAP.
