Healthy Hearts Program

As an integral part of Denver Health Cardiovascular Department, Healthy Hearts transitions patients from being in the hospital to managing their disease on their own at home. The program is conducted by a team of experts, including a nurse practitioner, an RN and two exercise physiologists who work closely with our cardiologists in managing patients with various heart conditions. 

Healthy Hearts provides education to all hospitalized patients with congestive heart failure, heart attack, coronary stent placement, coronary artery bypass surgeryvalve surgery or cardiac device placement. Healthy Hearts enables patients and their families to manage daily living with a heart condition by teaching them about cardiac medications, monitoring of daily weight, diet counseling, exercise, and smoking cessation.

Before an eligible patient goes home from the hospital, he or she is scheduled for a return visit within two weeks with Healthy Hearts. During that return visit a nurse practitioner or an RN and a cardiologist will review the patient's progress. Healthy Hearts facilitates enrollment into the Colorado “Quitline” program for smoking cessation and also strives to enroll patients in our Cardiac Rehabilitation exercise program.Get With the Guidelines Gold Award

Healthy Hearts follows the recommendations of the American Heart Association for guideline based care for congestive heart failure and heart attack patients. The American Heart Association recognized our Healthy Hearts program with its Gold Award in 2009, 2010 and in 2011 for excellence in the “Heart Failure Get with the Guidelines” program.
