Healthcare Program for Children with Special Needs

The Health Care Program for Children with Special Needs (HCP) provides care coordination for families that have a child from birth to age 21 years with a special health care need. Families may be provided with information and resources about numerous topics including:

  • The medical home/specialty service network
  • Support groups
  • Community programs/services
  • Information about schools and/or early intervention services

The goal of the program is to assist families in gaining knowledge about services and supports within the community, so that they are better able to coordinate their child's care with increased independence and confidence.

The Denver HCP office serves children and youth with special health care needs throughout Denver County.


Referrals can be made through the Denver Health Pediatric Referral System, in person, or via the phone.
