Primary Care Services
Primary care is an essential element for healthy living. Every patient should have a primary care provider who helps them stay healthy, manage any chronic medical conditions, and assist with diagnosis and treatment of minor medical problems. The primary care provider and the clinic team will provide you and your family with information to help you stay healthy, and with vital preventive treatments such as well adult and child care, reproductive care, vaccines ,and cancer screening. These providers can also refer you to specialists when needed.
All the Denver Health primary care clinics are certified as Patient-Centered Medical Homes and we encourage partnerships between individual patients/families and their personal providers in conjunction with the entire health care team, including our nursing support staff, care coordinators and outreach workers. The clinic teams have access to advanced information technology systems that will help us ensure that all or our patients receive the medical and preventive care that is needed when needed, and to coordinate care with specialists and the hospital as needed. All of the clinics provide routine care for on going medical problems and acute illnesses.
