Return the signed application, fee, and photo copies of the appropriate proof of identity/relationship documents (see this list for accepted forms of identification) to:

If you are unable to print the applications above, please send a written request including the following:
- Your full name
- Date and place of birth
- Parents' names (including mother's maiden name)
- Your daytime phone number
- Your return address
- A photocopy of your current, valid state-issued driver's license, state identification card, passport, or other acceptable identity document(s) from this list
Your request will require four weeks processing time after we receive it.

Phone requests are processed in three to five business days and have an additional $9 processing fee.
Next day service is available for an additional $19 processing fee (effective August 4, 2008).
