Birth and Death Certificates Minimize

Denver Public Health's Vital Records department provides certified copies of birth certificates for the State of Colorado and certified copies of death certificates for Denver County.

Vital Records is open weekdays from 8 a.m. – 4:30 p.m., and is located on the 3rd floor of 605 Bannock Street, Denver, Colorado 80204, 303-436-7350, the fax number is 303-436-7321. Walk-In customers need to arrive at our office by 4:00 p.m. for their request to be processed by 4:30 p.m.

Vital Records is authorized to issue:

  • Colorado birth certificates from 1905 to present
  • Denver county death certificates from 1964 to present

Fees

  • Certified Birth or Death Certificate, $17.00
  • Additional copies of the same certificate if ordered at the same time, $10.00
  • Correction Fee, $20.00
  • Paternity Fee, $20.00
  • An additional $9.00 fee will be added to the order when using a credit card, except when using our walk-in service
  • Net-Day Service vis UPS - $16.25

Fees subject to change at the discretion of the Colorado State Registrar.

Vital Records accepts cash, checks or credit cards as payment.

To obtain a copy of a birth certificate:

  • Name
  • Date of Birth
  • Place of Birth
  • Parent's names, including mother's maiden name
  • Your relationship or tangible interest to the person on the birth certificate

To obtain a copy of a death certificate:

  • Name of decedent
  • Place of death
  • Date of death
  • Your relationship or tangible interest to the person on the death certificate

Requests can be made:

All requests for birth and death records must be accompanied by a photocopy of the requestor’s identification before processing. We require the following documentation:

At least 1 of the following:
(All items presented must be current and valid. Expired documents drop to the secondary list.)                                               

* Expired documents may only be used as a secondary document if they have expired within six months of the current date.

Corrections: It is possible to make changes on a birth or death certificate under certain circumstances. Please call 303-436-7350 for additional information.

If the parents are not married at the time of the child's birth, the father's name will not be listed on the birth certificate unless a "Statement of Paternity" form is filled out. If the "Statement of Paternity" is completed while mother and baby are still in the hospital there is no charge.

Paternity forms can also be submitted to our office and processed after the child is born. The fee for this is $20.00. Please contact our office for additional information.

Vital Records cannot obtain certificates from other states. Vital Records can give information on how to obtain a certificate or contact other State Vital Records Offices.
