DH Medical Plan, Inc. Minimize

Created in 1997 as a not-for-profit HMO, Denver Health Medical Plan, Inc. (DHMP), concentrates its efforts on delivering quality products and services. The goal at DHMP is to provide the Denver community with access to high quality, cost-effective, locally managed health care to our customers' satisfaction. Because we too are residents of the Denver metro area, we offer a unique understanding of our members' individual needs and concerns.

In 2001, DHMP earned a Commendable Accreditation by the National Committee on Quality Assurance (NCQA). NCQA evaluates how well a health plan manages clinical and administrative systems to best serve their members. A commendable score demonstrates levels of service and clinical quality that meets or exceeds NCQA's rigorous requirements for consumer protection and quality improvement.

Health Plans
Denver Health considers the different needs of different people by offering the following options:

Child Health Plan Plus (CHP+)
Through the Colorado Department of Health Care Policy and Financing, Denver Health Medical Plan offers the Child Health Plan Plus, a low-cost health insurance program for children from birth through age 18.

Children living in the City and County of Denver, Jefferson County or Arapahoe County are eligible for the Child Health Plan Plus program at Denver Health. Once qualified for the program, children can be seen at any facility within the Denver Health system. For more information, call 720-956-2100 (or toll-free 1-800-700-8140). Denver Health Medicare Choice
As of January 1, 2006, Denver Health began offering a Medicare Advantage Special Needs Plan, with Part D Prescription drug coverage, called Denver Health Medicare Choice. To enroll in the plan, members be eligible and enrolled in both Medicare and Medicaid, receiving dual benefits, and reside in the City and County of Denver. Denver Health Medicare Select
As of January 1, 2007, Denver Health added another Medicare Advantage plan, with Part D Prescription drug coverage, called Denver Health Medicare Select. This plan is beneficial for those who do not meet Medicaid requirements but may qualify for CICP and/or LIC subsidy, and reside in the City and County of Denver. OTHER PLANS offered by Denver Health:

Denver Health Medicaid Choice is a managed care plan created to care for Colorado Medicaid patients. This plan offers members added benefits above those offered by Fee-for-Service Medicaid, such as over-the-counter medicine, immunizations and office visits at no cost with low copays for prescriptions.

DHMP Employee Plans Minimize
Health Plans for Denver Health, Denver Career Service Authority and Denver Employee Retirement Plan Employees
Denver Health Medical Plan offers three low-cost health care plans for employees of Denver Health, the City and County of Denver Career Service Authority, and the Denver Employee Retirement Plan.

Denver Health Medical Care offers members health care services at any facility within the Denver Health system, with low co-pays. Members have access to any of the nine Family Health Centers located throughout the City and County of Denver, as well as the Denver Health Medical Plan Clinic, located on the Denver Health main campus at 6th and Bannock, and dedicated solely to plan members. All health care facilities within the Denver Health system are staffed by highly skilled, caring medical professionals, many of whom are board-certified. All Denver Health physicians have faculty appointments at the University of Colorado School of Medicine.

Denver Health/Cofinity (formerly Sloans Lake) Point of Service offers members a variety of options when accessing health care. A three-tiered program, member may
  1. Stay within the Denver Health system and incur the lowest out of pocket costs;
  2. Use the Cofinity provider network with more than 5,000 health care providers. Self-refer to both specialists and primary care providers. Co-pays as well as co-insurance and deductibles apply.
  3. Go to a provider of the members’ choice, outside both networks. Co-pays as well as co-insurance and deductibles apply at the highest rate.

DERP Medicare
