Appointments

Travel Consultation

*If you need prescriptions for travel, such as Malaria, traveler's diarrhea or high altitude sickness medication, yellow fever vaccination or recommendations of vaccines for travel, you are required to make a travel consultation.

The Denver Public Health Travel Clinic is available to help answer any questions you have before your trip. Please make an appointment if you are unsure of what vaccinations or prescriptions you need.

Travel consultations are not always necessary. To get the immunizations you need for your trip, you may come to the Denver Public Health Travel Clinic on a walk-on basis. A walk-in visit does not include recommendations for vaccines, yellow fever vaccination or needed prescriptions for malaria, high altitude illness or traveler's diarrhea.

Appointments can be made by calling the Denver Public Health Travel Clinic at (303) 602-3520 or by email at travelclinic@dhha.org. Ideally, appointments should be scheduled six to eight weeks before your trip so you have time to complete needed vaccinations.

Please bring the following items to your consultation:

  • Photo ID
  • Your trip specific itinerary
  • Completed traveler's history form
  • Adult patient consent form (English) (Spanish)
  • Child/Teen patient consent form (English) (Spanish)
  • Copy of your previous immunization records with dates, if available
  • Your International Certificate of Vaccination (yellow card), if applicable

Please arrive 15 minutes prior to your scheduled appointment to ensure you have time to fill out all the necessary documents. The entire process, including consultation, vaccinations and administrative paperwork, will take about an hour. However, the length of the appointment will vary depending on the size of the group, number of countries visited, medical history and age of the patient.

If you arrive more than 15 minutes late for your appointment, we may need to reschedule, so please make sure you arrive 15 minutes early.

Fees

The Denver Public Travel Clinic does not accept health insurance. Payment is due at the time of service.  We accept cash, check, Discover, MasterCard, Visa and American Express. An itemized receipt of all charges will be provided to you. Denver Public Health is not responsible for billing of claims.

Service Fee
Travel Consultation Fee  $60 + Immunization and Prescription Costs
Travel Consultation for each additional adult in family/group $60 + Immunization and Prescription Costs
Additional Children* $25 for each additional child + Immunization and Prescription
*If a child under 18 is the only patient needing the consultation, the cost will be $60.
