Appointments

ATTENTION: There is currently a National shortage of Yellow Fever vaccine. Our clinic has limited supply. We are now requiring a consultation for anyone who needs a Yellow Fever vaccine. Please call us directly for more information at 303-602-3520. We are sorry for the inconvenience.

The Denver Public Health Travel Clinic is happy to help you answer any questions you have before your trip. Please make an appointment if you are unsure of what vaccinations or prescriptions you need for your trip.

Travel consultations are not always necessary. To get the immunizations you need for your trip, you may come to the Denver Public Health Travel Clinic on a walk-on basis. A walk-in visit does not include recommendations for vaccines needed or prescriptions for malaria, high altitude illness or traveler's diarrhea.

Appointments can be made by calling the Denver Public Health Travel Clinic at 303-602-3520 or by email at travelclinic@dhha.org. Appointments should be scheduled six to seven weeks before your trip.

Please bring the following items to your consultation:

  • Photo ID
  • Your trip specific itinerary
  • Completed traveler's history form
  • Patient consent form (English) (Spanish)
  • Copy of your previous immunization records with dates if available
  • Your International Certificate of Vaccination (yellow card), if applicable
  • $2 for parking fees (cash or coins only)

Please arrive 15 minutes prior to your appointment to ensure you have time to fill out all the necessary documents. The entire process, including consultation, immunizations and administrative paperwork, will take about an hour. However, the length of the appointment will vary depending on the size of the group, number of countries visiting, medical history and age of the patient.

If you arrive more than 15 minutes late for your appointment, we may need to reschedule, so please make sure you arrive 15 minutes early.
