Patients may qualify for our services if they have been hospitalized three or more times in the last year, or have two or more hospitalizations plus a major mental health diagnosis.
*Patients are screened Monday - Friday. Please have your provider place a referral in ARL to start the screening process.

The Intensive Outpatient Clinic at Denver Health helps patients with complex needs that may require additional assistance or resources to help optimize their health. Our clinic specializes in providing a greater depth of care for those who have been recently hospitalized because of severe medical or psychiatric illness.
With a multidisciplinary team of doctors, nurses, clinical social workers, patient navigators, mental health and substance abuse counselors, psychologists and psychiatrists, we have the resources in place to help each patient get the medical care they need to live a healthy life.
