If you would like to view your medical record online, effective for services rendered on and after April 9, 2016, please access Denver Health's new MyChart electronic records system. MyChart is a secure online tool that allows you to view your medical history, message your health care provider, pay your bill, make an appointment and request prescription refills ‐‐ all from your smartphone or computer.
We are happy to provide you with a copy of your medical record. To request a copy of your medical record, you, or someone you designate, must complete the Authorization to Release Patient Health Information form.

Medicaid is Covering More People - Enrollment is Open Year Round
Colorado Medicaid is low-cost health insurance that is available to individuals and families with limited income. This program now covers more people in Colorado with limited income.
At Denver Health, many of our patients who have the Colorado Indigent Care Program (CICP) may now qualify for Medicaid as well.
