How do I request a trip report?
Patient medical records, including paramedic trip reports, may be accessed via Denver Health’s Healthcare Information Management Department (HIM).
- Denver Health patients may request their records via their MyChart account at www.denverhealth.org/mychart
- Medical records requests may be entered into HIM’s web portal at www.denverhealth.org/patients-visitors/medical-records
- Email requests to DH_HIM@dhha.org
- HIM may be reached by phone at 303-602-8000. Customer service hours are Monday-Friday 8:00am-4:30pm
- Walk-in assistance is available Monday-Friday 8:00am-4:00pm, DHMC Pavilion G/Webb Building, 1st floor
A detailed explanation of medical record release requirements and procedures, as well as all applicable forms, is available on HIM’s website including authorization forms, records amendment requests, PHI information, etc. These forms are available in a variety of different languages.
Inquiries not directly related to obtaining medical records should not be directed to HIM, but rather to the appropriate department such as billing or paramedic administration.
How do I ask questions regarding my care, or send in a compliment or make a complaint?
General questions, compliments, complaints, etc. may be directed to DH Paramedic Administration by emailing Paramedics@dhha.org. You may also call 303-602-2566.
I have a question about my bill?
All billing inquiries should be addressed to Apex Billing at 720-486-0577.
Denver Health Paramedic Division cannot answer any questions regarding billing.

How do I request Denver Health Paramedics to provide medical coverage for an event?
The person, organization, or other entity applying for a special event permit in the city and county of Denver is required to contact DH Paramedics. DHPD will help the event organizer determine the appropriate level of medical coverage for the event.
