About Primary Care at Denver Health

All of Denver Health’s primary care clinics are certified as Patient-Centered Medical Homes, and we encourage partnerships between individual patients/families and their personal providers in conjunction with the entire health care team, including our nursing support staff, care coordinators and outreach workers. The clinic teams have access to advanced information technology systems that will help us ensure that all of our patients receive the medical and preventive care that is needed and to coordinate care with specialists and the hospital. All of the clinics provide routine care for ongoing medical problems and acute illnesses.

We also have locations that provide specialty care for obstetrics and gynecology, internal medicine, dermatology and family planning.

As part of a large health care system, our primary care physicians provide the resources to offer specialty care whenever you need it.

What is Primary Care?

In this short video, find out why it's important to see a primary care doctor and a few simple things you can do to live a healthy lifestyle.

Primary care is an essential element for healthy living. Patients are encouraged to have a primary care provider who helps them stay healthy, manage any chronic medical conditions, and assist with diagnosis and treatment of minor medical problems. The primary care provider and the clinic team will prepare you and/or your family with information to help you stay healthy and provide vital preventive treatments such as adult and child care, reproductive care, vaccines and cancer screenings. Our providers can also refer you to specialists when needed, either at Denver Health or outside of our network.
