Children and young adults up to 21 years old with complex medical needs are eligible for nurse level care coordination at their medical home throughout the Denver Health System.
We provide a specialized and coordinated approach to managing the medical, behavioral and social needs of children with multiple chronic medical conditions. Care is both family-centered and evidence-based and our team works hard to prevent unnecessary medical visits and hospitalizations.
Who Qualifies?
Children requiring home health assistance and durable medical equipment may be eligible. Diagnoses include:
- Extremely premature and very low birth weight infants and toddlers
- Cerebral palsy and other neuromuscular disorders
- Spina bifida
- Children requiring feeding tubes (any type)
- Trisomy 21 (Down Syndrome)
- Many other genetic and congenital diagnoses
- Children recovering from major trauma
- Children with multiple significant medical diagnoses requiring multiple subspecialists.
Complex Care Clinic (CCC)
For families that are interested, we also have a multidisciplinary clinic for these patients. It provides primary care or consults for patients that qualify. Our team members all have years of experience caring for children and youth with special health care needs.

Our team works closely with subspecialists at Denver Health and other area pediatric specialty sites so that our patients can lead the healthiest lives possible. Clinic visits are longer (1-2 hours) allowing each team member to assess the patient when needed.
Our Care Navigator can help with scheduling and often with transportation.
Accommodations can be made for children with special sensory needs (like low lights, fidgets, weighted lap pad, ear protection, and I-pads).
